Whether you need a single test or a complete panel, we provide a wide variety of options at discounted prices. How to order & What to expect

1. Submit Your Request

Using the form below, select the labs you wish to order, by choosing the type of test at the top tabs and then clicking the box beside the specific lab you wish to request. When you click on a lab it will automatically populate in the order section of the form below it. You may choose as many labs as you wish. 

  • Someone from our office will contact you within 1 business day to schedule a lab draw. If you have not heard from us within 24 hours, or 1 business day, please call us at 608-222-2700 to schedule your appointment.

2. Your Appointment:

Please arrive on time and ready for your labs ( fasting, etc.) Payment is due at the time of service.

3. Your Results 

Following your appointment, results will be uploaded to a secure patient portal for your view. You will receive an email notification when they are available. ​

What to Expect Next

  • A member of our laboratory team will contact you within one business day to schedule your lab draw. If you have not heard from us within 24 hours, please call our office at 608-222-2700.

  • After your appointment, your results will be uploaded to your secure patient portal. Most results are available within two weeks

  • Need help interpreting your results? A consultation with one of our providers is available for an additional fee.
